What product codes are impacted?
All products that were previously ordered from Teleflex Medical Incorporated and Arrow International, Inc. will now be ordered from Teleflex LLC. This includes brands such as Arrow, Deknatel, Hudson RCI, LMA,
Pilling, Rusch,
Vascular Solutions, Vidacare and Weck.

What is my Teleflex LLC account number?
Your account number with Teleflex LLC will be the same as your Teleflex Medical Incorporated and Arrow International, Inc. account number. For example, if your account number is currently 12345, your Teleflex LLC
number will also
be 12345. For more information, please contact us at

How do I place orders with Teleflex LLC?
For orders issued via
EDI
(electronic data interchange), the new identification number is
Qualifier: ZZ, ID: TELEFLEXLLC
or
ZZ / TELEFLEXLLC.

Orders placed before August 5th should be placed with Teleflex Medical Incorporated and Arrow International, Inc., just as you do today. Invoices issued by Teleflex Medical Incorporated and Arrow International,
Inc. should be paid
according to the instructions on the respective invoice.
Can I continue to place orders with Teleflex Medical Incorporated and Arrow International, Inc. after August 5, 2019?
Beginning on August 5, 2019, all orders should be placed with Teleflex LLC. Failure to place orders with Teleflex LLC may affect how quickly we process and deliver your order.
Does this change impact contract terms or pricing?
All pricing and contract terms in place with Teleflex Medical Incorporated and Arrow International, Inc. at the time of the change will transfer to Teleflex LLC.
There is no change to contract numbers or
pricing.
Will I need to keep Teleflex Medical Incorporated and Arrow International, Inc. active in my system?
Beginning on August 5, 2019, all orders should be placed with Teleflex LLC. Once all current invoices and deductions have been fully resolved for Teleflex Medical Incorporated and Arrow International, Inc., you can
discontinue
these vendors in your system.
Where do I submit my vendor setup form for Teleflex LLC to complete?
Will my Teleflex Medical Incorporated and/or Arrow International, Inc. sales representative(s) change?
No. The Teleflex LLC sales representation will continue to be the same as today.
